Hospital Palliative Care Team
Palliative care is an approach that improves the quality of life of patients and their families facing the problem associated with life- threatening illness, through the prevention and relief of suffering by means of early identification and impeccable assessment and treatment of pain and other problems, physical, psychosocial and spiritual ( World Health Organisation 2019).
We are a hospital based palliative care team. Our team includes:
Clinical Nurse Specialists
Care of the Dying Nurse
Advanced Care Planning Facilitator
Advanced Care Planning Co-Ordinator
We work closely with all clinical teams, the hospital chaplaincy team, cancer counsellors and the acute oncology service.
We also work with the wider health community and can coordinate referral to the local community palliative care teams and hospices for ongoing symptom management, advice and support.
The team supports conversations about advance care planning including wishes about preferred place of care.
The clinical teams in the hospital can refer patients under their care to the hospital palliative care team.
When the hospital palliative care team are involved in a person’s care a contact number will be provided. The team is available 09.00-17.00 every day. Outside these hours the clinical teams can access specialist palliative care advice from a local hospice.

The team can also provide other information, including Macmillan booklets, on various topics according to individual requirements.

The team is based in Regent Wing in the hospital grounds but meets patients and their families/friends either on the ward or in a dedicated private space.

Patients and/or their families can ask the relevant hospital medical team to make a referral to the Hospital Palliative Care Team
