Palliative care is an approach that improves the quality of life of patients and their families facing problems associated with life threatening illness, through the prevention and relief of suffering by means of early identification and impeccable assessment and treatment of pain and other problems, physical, psychological and spiritual". (World Health Organisation 2002).
We are a hospital based palliative care team consisting of specialist nurses and palliative care consultants. Our extended team includes a social worker and the hospital chaplaincy team. The team works in an advisory capacity alongside other health care professionals involved in the persons care.
The aim of palliative care is to provide a holistic approach to care focusing on issues of comfort and quality of life rather than cure.
When the hospital palliative care team are involved in a person’s care a contact number will be provided.
The service is available Monday-Saturday, 9.00-17.00.

The team is based in Regent Wing in the hospital grounds but meets patients and their families/friends either on the ward or in a dedicated private space.
How to refer:
Patients and/or their families can ask the relevant hospital medical team to make a referral to the Hospital Palliative Care Team
